The Benefits of Working at Friendship Village

Friendship Village of Schaumburg proudly offers its employees a host of comprehensive benefits in which to participate. Our plans allow employees to select the programs that are most beneficial to their personal needs, both in terms of quality and affordability.


Aquatic and Fitness Center Membership

For just $5 every other week, employees can purchase a membership to the Friendship Village new aquatic and fitness center. The fee will be automatically deducted from your paycheck and includes access to the 75-foot pool as well as an array of Nautilus and Precor machines and free weights. Employees also will have use of dressing rooms equipped with showers, lockers, toiletries and hair dryers. For more details on this benefit, click here.

Medical Care

The FVS Medical Plan is available to all regular full-time employees (working 30 hours or more per week). FVS offers managed care networks for all your health care needs. You have your choice of three medical plans: BlueCross and BlueShield BluePrint PPO, BlueCross and BlueShield HMO Illinois and BlueCross and BlueShield HMO Blue Advantage.

The BC/BS HMO Plans provide benefits to employees and eligible dependents at a lower or no cost basis. The primary focus of the BC/BS HMO Plans is preventive well child and adult care. Office Visits are covered at 100 percent after a $20.00 co-payment. The only cost for an overnight hospitalization would be a co-payment of $100 for the first 5 days of hospital services. (Services provided by a surgeon throughout your hospitalization are paid at 100%.) The BC/BS HMO plans do not have deductibles and claim forms are not required. To receive benefits under the HMO plan, you must select a Primary Care Physician (PCP) from the Plan's provider directory. This PCP who will provide or arrange all health care services for you and your eligible dependents.

The BC/BS BluePrint PPO Plan allows you to decide, at any time, whether to use a BC/BS PPO participating providers or non-participating providers. When you choose these participating providers from the BC/BS PPO provider directory, you receive maximum benefits paid at 90% and reduce your out-of-pocket costs. When you use non-participating providers, your benefits are paid at 70%, and there may be claim forms to file. Office Visits (in-network) are covered at 100% after a $20.00 co-payment. Hospital and surgical services (in-network) are paid at 90% after the $500 Individual and $1500 Family Deductibles have been satisfied. There are also Out-of-Pocket limits that apply.

Prescription

The Prescription Plan is available to all regular full-time employees (working 30 hours or more per week). The prescription plan co-pays, for HMO and PPO, are $15/$30/$50. There is also significant cost savings by using the . Mail Order Pharmacy. for any maintenance prescriptions.

Vision

The Vision Discount Plan is available to all regular full-time employees (working 30 hours or more per week).

Dental Care

The Guardian Dental Plan is available to all regular full-time employees (working 30 hours or more per week). The plan has an annual deductible of $50.00 per person with a maximum annual benefit of $1,000.00. You have two choices: (1) to select your own dental provider or (2) choose a dentist from the Guardian Dental Directory. Preventive services are reimbursed at 100 percent with no deductible and include two annual oral examination/cleanings. The plan pays 80 percent, after $50 deductible, for basic care such as fillings and major care such as root canals are paid at 50 percent also with a $50 deductible. All dental services are paid based on a 'usual and customary' fee structure.

Section 125 Flexible Benefits Program

Section 125 Flexible Benefits Program is available to all regular full-time employees (working 30 hours or more per week). Section 125 Flexible Benefits Program is designed to provide you with a tax effective way of paying for eligible health/dental/vision care.

Life Insurance & Accidental Death & Dismemberment

All regular full-time employees (working 30 hours or more per week) are eligible for Company paid Life Insurance and Accidental Death and Dismemberment Insurance.

In addition to Company paid basic life coverage, all full time employees have the option of electing Voluntary Life coverage. Optional Life coverage is available for employee, spouse and child(ren). The cost of this optional coverage is 100% employee paid.

Short and Long Term Disability Income (After 1 year)

FVS pays the entire cost of the disability income protection plan. The plan is designed to provide protection for your illness/injury be it short-term or long-term. It is available to full time employees (working 30 hours or more per week). Eligible employees receive 60 percent of their pay based on number of year of service. Benefits begin after an absence exceeds 5 days. Employees are eligible after 1 year of full-time employment.

Retirement Savings

The FVS Retirement Savings Plan gives employees the opportunity to fund their retirement with pre-tax. dollars. After sixty days of credited service and age 21 or over, employees are eligible to contribute up to 14% of their income to their 401k retirement fund.

FVS will match up to 6% of employees' contributions with 50 cents for each dollar the employee contributes. The matching contribution will begin after one year. Employees may select from ten different investment options and receive quarterly personalized statements showing account balances.

Paid Time Off

Regular full-time employees and regular part-time employees who meet the eligibility requirements receive paid time off based on years of eligible service as well as number of hours worked.

The purpose of PTO is to provide regular full-time and part-time employees with time off with pay for vacation, sickness, and unplanned absences. PTO hours are accrued every pay period.

Holidays

Regular full-time employees (working 30 hours or more per week) will be eligible for eight holidays per year. Part-time employees are eligible for 50% of the holidays.

Tuition Reimbursement

Regular full-time employees (working 30 hours or more per week), are eligible for up to $1000 for tuition reimbursement annually. Courses must be pre-approved by the supervisor, and an employee must achieve a grade of C or better, or a . pass. in a pass/fail grading system in order to receive tuition reimbursement.
